Risk Factors
- Tamoxifen
- Postmenopausal > Premenopausal
- Obesity(Because of high estrogen)
- Sedentary lifestyle
- Diabetes Mellitus
- Early Menarche
- Late Menopause
- Unopped form of Estrogen containing HRT
Types of Endometrial Cancer
- Pure Sarcoma
- Malignant Mixed Mullerain Tumor
- Endometroid Adenocarcinoma
Most common type is endometroid adenocarcinoma which accounts for about 60% cases of endometrial carcinoma.
Trials for the management of Endometrial Cancer
- ASTEC Trial
- PROTEC Trial
Investigations of Endometrial Cancer
- Transvaginal Ultrasound ( First line)
- Endometrial Biopsy (Pipelle , hysteroscopy , diagnostic DnC etc)
- Histopathology ( To check for hyperplasia with atypia and hyperplasia without atypia)
- MRI is done for stagging and invasion of metastasis.
With what endometrial thickness will we take biopsy for diagnosing Endometrial Cancer
Endometrial thickness cut off of 4 mm or more should be investigated as recommended by British Gynaecological Cancer Society.
Recurrence of Endometrial Cancer
- Most common site of Relapse :-
- Vaginal Vault .
- If previous Surgery only :-
- Radiotherapy is the treatment.
